The Employee Assistance Program offers solution-focused counseling services to employees and dependents of contracted companies. The goal of the EAP is to restore troubled employees to full productivity. Employee Assistance Program Fees and Service Fee for Service Agreement: $80 per hour for direct service (client contact – assessment, individual, marital, family, conjoint counseling). Inservice

Two (2) Year Agreement:
$15 per employee One (1) Year Agreement: $16 per employee 1. Assessment to determine existing
personal problems. 2.
A maximum of five (5) free sessions (assessment included) per year. 3.
Referral for treatment to appropriate provider. 4.
Follow-up to monitor status for treatment being provided and/or the quality of
job performance. 5.
Lawyer Referral Service. 6.
Assist company's EAP committee in promoting and evaluating services. 7.
Provide promotional materials (posters,
payroll stuffers, brochures) and presentations. 8.
Develop a liaison to EAP Committee. 9.
Provide orientation to all employees, as needed. 10.
Provide training sessions on use of the EAP for all supervisors / union stewards,
if company is unionized. 11.
Provide utilization statistics quarterly. 12.
Hold all employee records in strictest confidence and release
information identifying an employee or family member only on the written
consent of the employee or family member. 13.
Presentations on the following topics: a. Alcohol and Other Drugs b. Depression and Suicide c. Children of Alcoholics d. Self Awareness and Self-Esteem e. Treatment, Intervention and Referral
Sources f. Alcoholism g. Communication Skills h. Motivation and Positive Thinking i. Changes in the Workplace j. Stress Management k. Workplace Violence I. Empathy (Other
presentations may be tailored to the needs of employees.) CHR
